It is correct, but unclear because there is no punctuation. If you punctuate it like so: "I love you truly, madly, deeply, always sincerely, forever." always will be taken as modifying sincerely. If it is "I love you truly, madly, deeply, always, sincerely, forever." always will be taken as modifying love.

If you were reading it, you would pause on each comma.
The meaning of a sentence can change a lot with punctuation.
A panda walks into a café. He orders a sandwich, eats it, then draws a gun and fires two shots in the air.
"Why?" asks the confused waiter, as the panda makes towards the exit. The panda produces a badly punctuated wildlife manual and tosses it over his shoulder.
"I'm a panda," he says at the door. "Look it up."
The waiter turns to the relevant entry in the manual and, sure enough, finds an explanation.
"Panda. Large black-and-white bear-like mammal, native to China. Eats, shoots, and leaves."
While the following refers to the panda's eating young bamboo.
"Panda. Large black-and-white bear-like mammal native to China, eats shoots and leaves."
